Am I total lost cause ?

Am I total lost cause ? I have been dealing with my big toe issue for years , I've never wanted to take the oral medication due to potential liver issues plus I'm also of child bearing age. My other toes are starting to get infected now. I used the ariella pen from Amazon which causes some of the yellow/orange discoloration. My big toe has had so much trauma from things falling on it people stepping on it etc. Any hope ? Any recommendations appreciated ! I also am very sus
